WEMROCK BROOK MEDICATION DROP OFF SIGN UP AND PROCEDURES

Please complete the health update form found here: MERS Health Form and return via email or with student. It can also be faxed to 732-786-2610.
If your child has any Conditions requiring any daily or EMERGENCY medications, please see below:
Medication Policy: It is illegal for a student to carry ANY medication to or from school or while in school. (This includes over-the-counter medication such as cough drops, eye drops, allergy medication, Tylenol and Advil.)
Any medications (prescription or over the counter, including, but not limited to cough drops, eye drops, allergy medication, Tylenol and Advil) to be given during school hours must be kept and administered in the Nurse’s office with required paperwork received (MD orders and parent’s permission).
To access these documents, please go the district Nurse website, https://www.mersnj.us/domain/2424 and you will find all the packets for all your medication needs.

The Student Medication Order packet can be used for all over-the-counter medications.

ALL medications need a doctor’s order.

If your child will be requiring any medication to be given during the school hours for the 2023-2024 School Year, medication and all required paperwork must be dropped off at Wemrock Brook School during the following times:
9am - 11:30am
Please follow the following procedures:

Register your visit on Passage Point

Park in front of the school and ring the bell.

State “Medication Drop off” as the purpose of your visit.

Ms. Cammarata will come outside to meet you.
Medical Packets for Parents/Physicians
- Asthma Packet
- Diabetes Packet
- Epinephrine Packet
- Student Medication Orders
- Seizure Packet
- Adrenal Insufficiency Packet
Please print all the forms in each packet you require.
Return all forms from your required packet to your school nurse.
All medical packets are completed annually, allowing the forms to be valid for a full school year (July 1st - June 30th).
All forms must be dated on or after July 1st of the current school year OR clearly marked for the current school year (20_ _ - 20_ _).
