We are pleased to inform you that Health Forms can now be obtained and printed from the District Nurse page. 

Please click on the following link "District Nurse Page" to obtain and print health forms. 

On the District Nurses page, you will be able to access the following forms:

Medical Packets for Parents/Physicians

Asthma Packet

Diabetes Packet

Epinephrine Packet

Student Medication Orders

Seizure Packet

Adrenal Insufficiency Packet

Please print all the forms in each packet you require.

Return all forms from your required packet to the school nurse.

All medical packets  are completed annually, allowing the forms to be valid for a full school year (July 1 - June 30th).

All forms must be dated July 1st of the current school year OR clearly marked for the current school year (20_ _ - 20 _ _ ). 

Registration Forms for Incoming Students

Universal Child Health Record (Entrance Physical Exam Form) 

NOTE: The Universal Child Health Record  (along with a copy of your child's updated immunization records) needs to be completed for all students upon registration into the Manalapan-Englishtown Regional School District.

Student Athletic Forms

NJDOE Student Athletic Forms Letter

The Preparticipation Physical Evaluation Medical Eligibility Form needs to be completed annually and is needed for any sixth grade student who plans to participate in Pine Brook intramural sports.

The Health History Update Questionnaire is to be completed prior to the start of each intramural sport for any student whose sport physical examination was completed more than 90 days prior to the first day of the sport. The health history update questionnaire is to be completed and signed by the student’s parent or guardian. The Health History Update Form is included in the sign-up for each intramural sport. 

**Please print and complete any form(s) that may be applicable to your child and return them the Nurse's Office at your earliest convenience. If you have any questions regarding which health forms are needed for your child, please contact the Nurse's Office for assistance.
