Kindergarten Registration Information

All students entering kindergarten or first grade for 2018-2019 and are not currently enrolled in the district need to be registered for the 2018-2019 school year. Children must be born on or before October 1, 2013 to register for Kindergarten and on or before October 1, 2012 for First Grade.

You can save time by completing the registration online. Between Wednesday, December 20 and Friday, February 2, 2018, send an email to In the email, include your name, the child's name, and your relationship to the child. The district will send you a login, password, and instructions on how to complete the online registration. You then only need to bring the required documentation, outlined on our website, to the Wemrock Brook School on either Tuesday, February 27th or Wednesday, February 28, 2018.

For those who do not register on-line, registration will be held on Thursday, March 1 at the Wemrock Brook School. Please print out the forms from our website (CLICK HERE to access forms) and have them ready when you register. Please do not bring your children.

Please bring the following required documentation.

The original birth certificate and a copy. Only a certified birth certificate from the Bureau of Vital Statistics with a raised seal will be accepted. A hospital notice of birth or a photostatic copy is not acceptable.

Original and a copy of two proofs of residence (e.g., driver's license, utility bill).

Proof of Physical:
State statute 18A: 40-4 states all Kindergarten students and new students entering First Grade will be required to have physicals completed before they are admitted to school. Physical examinations performed on or after September 1, 2017 will be accepted. Forms are available in each school health office and online.

Immunization Records:
Four doses of diphtheria and tetanus toxoids and pertussis vaccine (DPT), one dose of which shall have been given on or after the fourth birthday. Alternatively, a child with any total of five doses of DPT type vaccines will also be in compliance with this regulation.
Three doses of Inactivated Polio Vaccine (IPV), or Oral Polio Vaccine (OPV) or a combination of both, one dose of which shall have been given on or after the fourth birthday. Alternatively, a child with any four doses of polio vaccine, spaced by a minimum of one month (twenty-eight days) will be in compliance with this requirement.
Two doses of a live measles-containing vaccine, or any vaccine combination containing live measles vaccine, such as preferred measles, mumps, rubella (MMR) vaccine, given after age 1. The second dose must be administered no less than one month from the date of the first measles-contained vaccine.
German measles (rubella) and mumps are required after the first birthday.
Hepatitis B vaccine. A three dose regime is required. Have your child's Hepatitis B status updated and bring the appropriate documentation.
Varicella (Chicken Pox) vaccination or written laboratory evidence of immunity. A physician's or parent's written statement verifying previous Varicella infection will also be accepted. Please include specific date.
Students transferring from another country must have a documented Mantoux skin test within the previous six months.
All students transferring from another state or country must follow Monmouth County TB requirements.

The doctor's signature must be on the immunization records. If your child was not immunized against mumps and/or regular measles because he/she had the disease, documentation signed by a physician is required. Children will not be assigned to a class in September until all records are complete. (Please bring a copy for us to keep on file.)

We recognize that students may not have all necessary immunization by the registration date.  We will still register students under provisional admission. Documentation must be received before September 1st or your child may not be permitted to attend school.

In addition to the required documents, online registrations must bring the media release form. In person registrations must bring all required forms. Please click on the link in the menu bar on the left of this page. Additional copies will be available at registration.

To explain our program to parents/guardians of all incoming Kindergartners, we will have a presentation by members of our administrative staff on Monday, May 14, 2018 at 7:00 p.m. in the Clark Mills School. Your children should not be in attendance at this meeting.
